We're searching for the people, projects, and initiatives from Australia's primary health care workforce deserving of recognition and reward for outstanding performance.
Help us to find and reward them. Make a nomination by 14 August 2011 at Primary Health Care Awards .
There are three categories which cover the wide range of professions and skills that Australia's primary health care personnel exhibit, sometimes in trying circumstances and environments. If you know outstanding contributors to primary health care, recognise their innovation and leadership by nominating them for one of three awards: Young Leader, Individual Distinction, or Team Excellence.
Nominations close 14 August 2011
Who can enter?
Australia's primary health care services are the first we turn to when we are sick or injured. Their care supports our families, friends and us every day.
Patients, carers, colleagues, and primary health care workers themselves can nominate a special individual or team who has made a real difference to the lives of patients or to the quality of a service. Everyone involved in every stage of the delivery of care can enter or be nominated.

A spectacular event
Winners will be announced at a prestigious gala dinner at Melbourne's Crown Entertainment Complex on Wednesday 16 November -- it promises to be an exciting celebration. To attend the event visit AGPN National Forum.
